Important Medical Information for First Year Student-Athletes

Dear Parents and First-Year Student-Athletes:

On behalf of the Sports Medicine Staff, welcome to Lasell University and congratulations on becoming a member of the Lasell Community. We strive to provide the highest quality Athletic Training services for our student-athletes. The first step toward doing this is to understand your medical history.  It is also important that we have each student-athlete's medical insurance information on record in case of injury, illness, or emergency that requires medical services beyond those we provide on-campus. 

All of your health information, including your medical history, emergency contact information, medical consent forms, and insurance information must be submitted electronically through our online Sportsware system.  Instructions on how to register and input this information are found here:

Also, all NCAA athletes are required to have a general medical physical by their primary care physician. The NCAA requires that this physical must be done within 6 months of initial participation. There are NO exceptions to this rule. Your physical form MUST be dated after March 1, 2021, in order to participate this year. The physical form can also be accessed from the link above. 

THIS INFORMATION IS IN ADDITION TO THE HEALTH FORMS THAT YOU WILL RECEIVE FROM THE UNIVERSITY AND SHOULD BE SUBMITTED SEPARATELY!

The Sportsware registration must be completed, prior to August 1 and the signed physical form must be returned to the Sports Medicine Office no later than August 1, 2021. You will NOT be able to participate in athletics until all of your paperwork is properly completed and verified. Submitting paperwork late may affect your eligibility to try out or to participate with a team.

Finally, in addition to submitting your medical information and the physical form, all athletes are required to complete an online concussion education training.  You must print your completed certificate and return it to the Sports Medicine office. This link is also included with First-Year Athlete Information on the athletic training website.  

Your signed physical form and your completed concussion certificate can be faxed to 617-243-2297, emailed to cnoyes@lasell.edu, or mailed to Chris Noyes, Sports Medicine, Lasell University; 1844 Commonwealth Avenue, Newton, MA 02466.
